TLDR : Answer Summary
An Iranian individual currently holding a Non-O visa in Thailand can potentially switch to a Non-B visa if he wishes to obtain a work permit. However, if his Non-O visa is based on being married to a foreigner with a work permit, he may not be eligible to switch. Typically, one would need to leave Thailand and apply for a Non-B visa at a Thai consulate abroad, or convert from a tourist visa to a Non-B visa upon re-entry.

Okay, IF he has an extension based on being married to a foreigner who legally works here (trailing spouse) he CANNOT work, AND I have never seen that extension reason be allowed to be changed to one based on employment inside the country.
He would have to cancel this extension, leave the country, and either get the Non-B from a thai consulate OR come back in on a tourist visa and apply at the immigration office to convert the tourist visa to a Non-B.
What type of Non-O visa does he have? If he has a Non-O for marriage to a Thai wife, he can hold a work permit at the same time, and there is no reason to convert to a Non-B
