Can we extend a 90 day, Non o, multiple entry marriage visa by reporting at Phuket Immigration? Or do we need to leave the country?

, you need to apply and get 1 year extension to not leave Thailand and do 90days report. With only your current multiple entry visa, you need to leave Thailand every 90days.

Can't you get a 60 day extension at the IO based on married to a Thai?

There is a stamp inside your passport with Admitted until ../../.. You are allowed to stay until that date inside Thailand and that is a period of maximum 90 days. No need to report that you stay 90 days or more inside Thailand.

You apply extension to your entry on Non-O for 1 year based on being marriage as Robert explained above or 60days visiting Thai family...

Or if you are asking about how the multiple entry visa works, you need to leave the country to be stamped for 90days again.

You can apply for a 1 year Extension of Stay at a local immigration office or if the valid until date on the visa is still in the future you can leave the country, come back and get a new 90 day of stay.
