I will be back in states in Feb and plan on applying for a non immigrate O-A long stay visa based on retirement . Will apply at LA consulate . One of the requirements is a police certification showing no criminal record from an Authority of my residence . Has any gotten this , no form provided . Can I use local sheriff or the Washington state patrol ?

Non O-A(Long stay/Retirement) needs the following documents:

Passport,

3 visa application copies,

3 passport size photos,

800k in Thai bank-3 months prior

or letter from embassy stating 800k per year,

Police certification,

Medical certificate(notarized).

Am I missing anything?

1) Does the medical need to be notarized by my embassy, or will immigration accept the clinic/hospital stamp?

2)Will immigration accept a medical certificate from a clinic?
