If I apply for a re-entry permit at the airport before I leave, will they ask for a 90 day report receipt? (90 day report will be overdue when leaving) AKA, if I’m leaving and coming back and my 90 day report is overdue, will I have trouble getting a multiple re-entry permit?

When you return the 90 days will start anew...but when you need to report your new 90 day report, expect to be required to do it in person and with a 2,000 Thai baht fine for not making your prior 90 day report timely.
if he is denied due to owing the fine then best to forget about it until the next extension so at least he will have the bonus of not having to bother reporting until then.
