TLDR : Answer Summary
A new COVID extension visa in Thailand will start from the date of application, and if it's their first extension, any remaining days on the current visa will be lost. Subsequent extensions, if available, will be added to the existing visa stamp.

If it’s their first covid extension then it will dated from the day they apply and they’d loose any remaining days they had currently. Subsequent ones (if still available) would be added to the existing stamp.
I went to MTT got my first covid extension and it's 60 days from the last day of my 30 days extension. I guess it depends on the immigration officer/ office
