Where should my Pakistani husband apply for a DTV visa, and can he use a visit visa to apply from that country?

A quick question: My husband is Pakistani and needs to apply for DTV visa, which country/embassy is best to apply from? And can we go on a visit visa to that country or it must be a residence country?
