first report after entering Thailand everytime needs to be done in person at the local immigration office. Subsequent reports can be done online. However, if you leave Thailand and enter again then you need to go to immigration again to do the report in person.
They will give it to you at Immigration when you show that you have all the documents you need for your 90 day report. It's a simple form you can fill out there when you get it.
Everywhere I have completed the 90 Day I get the form before showing immigration anything. Why wait in a queue twice? Each office has all forms needed out in the open to use.
The form is downloadable too but I have never bothered. In person I have only done the report in Jomtien and Chiang Mai. They are both very easy and quick.
