The bangkok immigration website is up running and answering where you can book appointments dirt Bangkok. The useless online 90 report is probably still down. :-(
Jomtien, Passport, TM30, TM6, fill in the form they give you. Subsequent 90 day reports just need your passport with the previous report receipt that was stapled in your passport.
TM 30 original receipt if it is your first time or previous 90 report receipt. TM6 (plane arrival card you filled and gave to immigration officer, the small square that he gave you back in your passeport)
