When does the 6-month tourist visa start: upon entry or issuance?

Question about the 6 month tourist visa. When does the "6 months" begin? When you enter the country or when the visa is issued? Just wondering how far in advance I should apply. Also, I understand you need an outbound ticket for exiting the country? Does that outbound ticket have to be for the first 60 days? 90 days? or 6 months? Thanks for any clarification.

A 6 month Multiple Entry Tourist Visa is valid for 6 months from the day it is issued by the embassy.

In order to qualify for METV, some embassies require proof that you need multiple entries. This means a flight into Thailand, a flight out of Thailand within 60 days of arrival, and another flight back into Thailand.
