What should I put in the application form for temporary stay in Thailand?

Hi guys, I am I am looking for some help with my Visa. I am dyslexic but I am trying to do myself I have got most of them all the paperwork ready but I am stuck a couple of times on the application form of temporary stay ￼ on the top of the form it says written at what do I put here and at the bottom of the form? It says I hear by apply for an extension period of temporary state in the kingdom for?this is where I am stuck again being dyslexic doesn’t help very much but I am trying. Is anyone willing to help me and check my form before I go to Immigration? I would happily pay or buy someone a coffee. Anything I am by BigC thank you.

Martin, written at they are looking for the office you are applying to they expect you are filling the form in at the immigration office e.g. Changwattana. The temporary stay for, they are asking why are you applying for the visa. Education, retirement, work etc.
