What should I do about booking a quarantine hotel and submitting Thai Pass documents for my girlfriend?

Sent of for mine and my girlfriends thai pass and have a few questions.

The booking of the aq sha+ hotel is booked under my name but it does say for two people, or would I be better of sending a screen shot of where it says additional person with her name?

Also if anyone has the axa insurance did they send “the schedule” which was in thai, or the certificate that was in English?

Once I sent of the documents for my girlfriend she never received a confirmation by email that it was received should I re apply for the pass?

Also if I think I made any mistakes ie from the questions above, can I re apply for the thai pass without waiting for the first reply lol? TIA

I think another shaba certificate could be better to proove the pcr payment for her

It can be booked under your name, but contact the hotel and ask for a SHABA certificate. They'll then ask for the Names/passport info for the people staying

I don't know about that insurance specifically, but I just sent in the basic coverage letter (1 page outline with names of covered people and general policy info) NOT the actual policy document.
