What type of hotel proof is required for the Thailand Pass application?

For those of you that have successfully received your Thailand pass, what kind of proof did your hotel provide?

I am hearing that some SHA+ hotels provide a SHABA form with a SHABA reservation ID whereas some other SHA+ hotels just provide a confirmation letter with a normal reservation ID.

Is either one valid?

Also, did you have to provide any specific proof regarding the inclusion of a PCR test in your SHA+ package, or was the letter/form from the hotel sufficient?

I will submit my application tomorrow and would like to make sure that I have the correct paperwork.

I just used my Agoda booking confirmation.

We did get a confirmation number which was put into the system….didn’t get a test confirm but we had one earlier from PSAS.

You need 'booking confirmation' from the hotel where you're going to test and go or quarantine/sandbox
