I’m applying for a multi non imm O in UK. I don’t recall the medical certificate necessary before. I have had this visa before. Is it necessary and how did you fulfill the requirements in UK?

You might be thinking of a multi-entry non-O visa. That is no longer available for quite a while now since the e-visa system became available.
The only multiple-entry retirement visa available now is the non-OA visa which requires much more than the non-O visa requires, including mandatory insurance, a health exam, a criminal background check and other items.
Most people get the single-entry 90-day non-O visa and then get a 1-year extension once they are in Thailand.
