What is the correct procedure for my husband's first extension of stay under a non-immigrant O visa in Thailand?

I'm on an extension of stay for my non-immigrant B visa (teacher, 1 year stamp in early May). My husband arrived on an 90 day non-immigrant O visa to accompany me in March, and we're applying for his first 1 year extension of stay. I have two questions:

Is the TM 7 Application for Extension of Stay in Thailand the correct form to fill out?

When we state the length of extension, should it be written to expire the date my extension expires (early May, less than a full year) or written to expire 1 year from the date of application?
