What is the best way to obtain a long-term stay visa for Thailand as a tourist returning for dental work?

Can anyone confirm if I have this right..

I returned to uk in August this year after 7 weeks in Thailand.

I'm returning in January for 4 months.

I'm a tourist and finishing off dental work there that I started earlier this year.

Do I get 90 day visa then go to the local immigration office and extend one month ?

If not, What's the best,/correct way to do this.

Apply for a one year medical visa… you can search specifics online. Similar to retirement visa in that it is an initial 90 days that can be extended for up to one year, but doesn’t have the bank account requirement.

I take it you do not have Thai Family (wife or child) to qualify for the Visit Thai Family 60 day extension?

Suggest if you need to be there for 4 months is apply for a non o over 50 retirement visa which is valid for 90 days then before it expires leave to a local country and then enter again on visa exempt for another 60 days. Having no 30 day extensions gives you a better chance of reentering without local immigration hassle

A 90 day visa is a non immigrant visa. They are only extendable for a year if you meet the requirements.

A tourist visa is valid for 60 days but then so is a free visa exempt entry if your passport qualifies. Both are eligible for a 30 day extension (for most countries).
