TLDR : Answer Summary
When you leave Thailand, the immigration officer takes your departure TM card (usually TM6). Upon your return, you will fill out a new arrival TM card, which the immigration officer will keep. They also stamp your passport indicating your re-entry.

Before you arrive at an immigration checkpoint you fill in the arrival /departure card. The arrival part is taken by the Immigration Officer and the departure card is saved with your passport, On departure the Immigration Officer takes this card. If you like to come back into Thailand, just start reading again.

No, with the Re_Entry permit you write the re-entry permit on the arrival card to get the same Admitted or permitted until date as you have now. But he arrival/departure card will be a new one, that is also why many Immigration Offices want you to visit them after being abroad to update your address registration with the new tm6 departure card number
