What forms do I need to complete for my move from Rayong to Bangkok while on a retirement visa extension over 50?

Hello, am moving permanently to Bangkok from Rayong province ... am currently on yearly extension of stay based on being over 50 ...

moving into a rented condo..

which forms need to complete. And take to immigration TM28 or 30 or BOTH .. and do I lodge this in Bkk when arrive or do prior at Mathaput?
