What documents are required to complete the TM30 registration for a house in Thailand?

I just successfully created an account on the TM30 site. However, when entering the address details for the house, it is asking for 2 items to be attached before it will allow the address to be saved. I assume 1 or both are pages from the house book but can someone be more specific?

it's in Thai and the translations indicate 'population' and something else. Another copy of the id and the house book makes sense as that is what they want at the IO to prove address. Thanks for responding.

In isn’t the tourist who shall make the TM30, it is the host of the house / hotel who have the obligation to make TM30, when a foreigner stay in their house.
