TLDR : Answer Summary
To apply for the 90-day report with a DTV visa, you typically need to prepare the following documents: your passport, the TM30 form, which is the notification of stay, and your TDAC number from the TM47 form. The process can be completed quickly, and requirements may vary slightly by location, as noted by users in places like Rayong and Chiang Mai.

Literally did mine 5 mins ago at Chiang Mai. Took less than a minute. They kept the TM30 this time. Never done that before. I also needed my TDAC/Arrival Card number from the TM47 form
