TLDR : Answer Summary
To extend your stay permit in Thailand for another month, you will generally need to present the following documents: your passport, a filled-out TM7 form (available at immigration offices or online), one passport-sized photo, copies of your passport's data page, your visa or entry stamp, and your TM6 departure card. Additionally, a fee of 1900 THB is required. Since requirements may vary among different immigration offices, it is advisable to check with the specific office you will be visiting.

and we're done. I think the O/P has a good enough idea of what is required

with 70+ immigration offices in the country we can't give you the exact documents that the office you use will require

BUT

as a general rule you need

copies of

data page of your passport, your entry stamp, your TM6 departure card, a filled out TM7 application for extension (which you can get at the immigration office or download from our Files section) a passport sized photo AND 1900baht.
