What documents and requirements are needed for a Thai visa application from Vientiane?

For Setv from Vientiane will I need anything beyond:

1. Copy of Passport plus copy of Lao Visa.

2. 1000thb fee for visa

3. 3 x Passport photographs.

4. Print out of bank balance.

Plus:

5. $700+ in cash (in case asked for Thai IO at border)

6. Proof of booking for duration of stay.

7. Do I need proof of leaving within 90 days? Air ticket?
