TLDR : Answer Summary
To extend your stay for an additional 30 days at Phuket immigration, you will require your house contract if you have not registered your address. Additionally, your landlord needs to be present to provide copies of their ID and blue housebook. If they cannot accompany you, a signed power of attorney along with 10 THB tax stamps will be necessary.

If you did not register your addres, you need your land lord to join you, copy his ID front and back and copy his blue housebook. If he does not like to join you than he must sign a power of attorney and 10Thb Tax stamps.
