TLDR : Answer Summary
To complete the 90-day reporting in Thailand, you will need your passport and your address details. Additionally, you'll need to submit a completed TM47 form to the Immigration officer. They will likely check for a TM30 (address notification) as well. No photographs or photocopies are necessary. After the process, you will receive a new loose leaf 90-day receipt in your passport.

You'll need a completed TM47 to give to the Immigration officer. You need to produce your Passport and they are likely to check you have a TM30 (address notification).

No photographs or photocopies needed.

You'll leave with what you took, (except the TM47) and you'll have a new loose leaf 90 day receipt in your Passport.

Usually painless :)
