What are the requirements for applying for a tourist visa in Thailand from Penang?

Hello, I have a few questions for people applying for a tourist visa in Penang. I've searched the group but I'd like to be sure 🙂

1. 1.Do I have to show the account balance from the last three months or is it sufficient to present a statement showing the current account balance?

2. Is a bus ticket to enter and leave Thailand ok or is it necessary to have booked flights?

3. Does the booked accommodation have to be for the entire 60 days or is the first week or month enough? Screenshot from Agoda or Booking is ok?

4. 4. Does anyone know any recommendable agency that organizes visa run from Koh Phagan or Koh Samui?
