What are the requirements for obtaining a SETV in Penang for 60 days?

Please, who has recent experience in Penang? I'm going to Penang to get a SETV for 60 days. It will be 3rd Thai visa in a row, all obtained from different countries. I worry about two requirements: 1) "..bank statement certified by the bank." - what do they mean by Certified? Do I need an original bank stamp on a statement or supporting letter from my bank? Usually I only print out the statement and now I'm not sure is it enough.

2) "Copy of reservation/confirmation of accommodation for the whole stay in Thailand" - I am going to stay 50 days in Thailand, do I really need bookings for 50 days? Is it not enough to show first week or first month?

I am ok with all other requirements, I will go by air both ways and I will have 20k with me. Thank you in advance!
