Hi everyone!

Are these the requirements for a re-entry permit? 

1. Passport with a valid Thai visa

2. Copies of the photo page, current visa page, and immigration departure card

3. One 2″ x 2″ passport photo

4. 1,000 baht for a single re-entry permit 

And...

Can I get a re-entry permit on behalf of my son without him being present?
