What are the requirements for a re-entry permit in Thailand, and can I obtain one for my son without his presence?

Are these the requirements for a re-entry permit?
1. Passport with a valid Thai visa
2. Copies of the photo page, current visa page, and immigration departure card
3. One 2″ x 2″ passport photo
4. 1,000 baht for a single re-entry permit
