What are the requirements and best practices for applying for a DTV visa under the soft power category for orthodontics in Thailand?

Has someone recently got their DTV under soft power category (orthodontics)? If so, would appreciate any suggestions on which embassy to apply from in SEA and would a letter covering 2 years of treatment plan along with diagnosis(3D intraoral scan records, clinical dental photos, digital X-rays) doctors ID, clinic registration suffice?

Will I have to translate the bank statements to English?

What other documents will need to be submitted? Where can I find the official checklist for this specific category?

I had an ED visa which was approved in March this year but the immigration did not want to extend it for another 3 months they recommended to switch to DTV and gave me a one month extension instaed. I have started my orthodontic tretament recently. I'm considering to do all the necessary paperwork and apply for DTV to complete the procedure
