1. What is the Certificate of Vaccination and some details about it?

1. Can anyone point me to where I can find the requirements for a Single Entry Tourist Visa for Thailand?

The Thai sites I've look up (including the Thai Embassy in US) don't load or are down.  Thanks very much in advance.  :)
