Hi all.I have a question regarding visa on arrival. In 2019 we needed to fill an application form , tickets, hotel booking and a photo. As I can see now , neither application form non a photo required any more. Can anyone confirm it?

Your profile says you're from Georgia. If that's the case then you would indeed qualify for visa on arrival. I do believe that requires filling out a form but unfortunately I don't know exactly what's required on the form. Hopefully someone else who has recent visa on arrival experience can answer.
