Apparently it is only available, very difficult to get, at the airport branch. There is one dedicated DTV guy, apparently, and from having a look through the posts, I don't think the results have been positive.

If I was you, I would go down to the airport branch and just ask for the requirements about a week before your date is due. They are very helpful down there, much better than the two other offices I dealt with for years in other matters.

Go late afternoon, 3 pm, when they're less busy. Then at least you will know up front instead of having to go waste time by sitting and having it rejected. Good luck. Let us know how it goes.

I have all the documents to show. That wouldn't be a problem, but if they still reject it even with the documents, then it would be a problem because I don't want to waste time

Do yourself a BIG FAVOR and leave Thailand and re-enter to get the New 180 day stamp. If you are in Chiang Mai, suggest you use an experienced BORDER BOUNCE service company to take you across the border and return the same day. There are several reliable BORDER BOUNCE service companies in Chiang Mai.
