It must be a legitimate need for medical care as deemed by your doctor and hospital. With written request to Thai Immigration from the doctor requesting an extension of stay up to 90 days based on your needed medical care. Requests of 90 days are usually questioned requesting added documentation from the doctor and the hospital.

There is NO such thing as a "medical visa". Inside thailand what you'd get would be an extension of stay up to 90 days at a time based on medical reasons. Some offices are super hard in issuing them and without a treating doctors statement and support documentation from the hospital they won't issue them.
