Is the old TM8 form still valid for obtaining a Re-Entry Permit in Thailand?

I need to go to immigration to get a 'Re-Entry Permit'. I've got an old blank TM8 form but I'm not sure if this is still the current form, attached. I've checked 'Forms', in this group and can't find a copy of a TM8 to check my copy against.

Can someone please comment if my TM8 form is still valid?

TIA

TLDR : Answer Summary
A user inquired about the validity of an old TM8 form for obtaining a Re-Entry Permit in Thailand. Comments revealed that while the old form may still be accepted, the current version is now a two-page format which does not indicate the re-entry fee. One commenter provided a link to the updated TM8 form and shared their quick experience at the immigration counter.

that is actually the OLD form (they will accept it but it's now a 2 page (one paper front to back) one that doesn't show the price of the re-entry

This should jump you to the PDF of the new form

Passed by a couple of weeks ago and it was only the IO waiting. No queue, and I would say maybe 20 min to be sure.
