Is the old TM8 form still valid for obtaining a Re-Entry Permit in Thailand?

I need to go to immigration to get a 'Re-Entry Permit'. I've got an old blank TM8 form but I'm not sure if this is still the current form, attached. I've checked 'Forms', in this group and can't find a copy of a TM8 to check my copy against.

Can someone please comment if my TM8 form is still valid?

that is actually the OLD form (they will accept it but it's now a 2 page (one paper front to back) one that doesn't show the price of the re-entry

I brought my own pictures and it took less than 5 minutes. No wait at all

got mine in about 15 minutes 1200 baht, 1000 for single entry permit and 200 for processing. They do it all, pictures and form. only a couple questions.

Passed by a couple of weeks ago and it was only the IO waiting. No queue, and I would say maybe 20 min to be sure.
