The health declaration form is contingent upon the countries a traveler has visited recently rather than their nationality. If a Bangladeshi traveler has been in countries that require a health declaration or a yellow fever certificate, they must provide that information on the TDAC form.

When you fill out the TDAC it asks you what countries you have been in recently. If one of those countries require a health declaration/copy of yellow fever certificate a seperate section will open up. Its countries you have visited not what passport you hold
