the op says it took only 15 max. I flew out on 23rd and it took me over an hour!!! I recommend reserving more time for the procedure

At the airport, you pay a +200 baht "service" fee, only you need is your passport to get a re-entry permit. They take care of photo and copies.

If you bring the form with a photo glued and copies of your passport, you don't have to pay an extra fee.
