How do I fill out the information for my E-Visa if I don't know exactly which day I am traveling back on? I need to get my visa back to figure out the rest of my trip and the LA Embassy is not accepting walk-ins like they did before and say it will take 15 days to get my visa back. ????

Put the date you think you'll be returning. Travel dates arent written on the visa. It's valid from the day it's issued.

Usually when applying for a visa you don’t need a confirmed (or paid for) flight, just an indication of when you are thinking of flying. Once you have the visa then you need confirmed flights and ASQ and insurance to get the COE. I do recall a couple of reports of some consulates insisting on showing confirmed flights but don’t recall which ones.
