Do I need to upload the entire 3-page declaration for a 60-day single-entry tourist e-visa for Thailand, or just the last signed page?

For a tourist 60 day single entry do you have to upload the 3 page declaration or just the last page with your signature?

I am applying for the e visa tomorrow and hope I get the visa by Oct 19th?

TLDR : Answer Summary
When applying for a 60-day single-entry tourist e-visa for Thailand, you should upload the documents as required. One comment suggests uploading only what is specifically requested, while another notes that their declaration was only one page. Therefore, it seems you may only need to submit the last page with your signature unless instructed otherwise.
