TLDR : Answer Summary
For the drive-thru 90 day reporting in Chiang Mai, you primarily need your passport and a current TM30 form. If this isn't your first report, you can also opt for the quick online reporting option which takes about 30 seconds to complete.

The new online 90 days is 30 seconds after you log on.

As long as your TM30 is up to date all you need is your Passport.

That's what I thought but just wanted to check

Last time I went there I had everything that the internet told me I needed such as passport photo all the forms filled copies of everything

Had parked my car across the street

Then they told me at the desk I need to go through the drive-thru so I walked out there and got between two cars

Then when I got to the window all he asked for was my passport and my TM 30 🤣🤣
