Depends on the office you use. Some like Phuket and Chiang Mai have drive through windows where you only need your passport. Some will want you to fill out a TM47 form and a copy of your passport photo page and last visa stamp.

Yes. At Chang Watana they asked for TM47, TM30 and copies of passport information and visa page. I Booked an online appointment beforehand which helped.

If you’re doing it in CW then you’ll need to fill out a TM47. If you got your extension in CW then you won’t need a TM30 as you already provided one. If you got your extension elsewhere then you’ll need a TM30 filed for Bangkok.
