Visa on arrival is not 30 days, it's 15 days. You are probably asking about visa exempt which means no visa at all.
No there is no application or forms, you just hand your passport and boarding pass over at passport control and they'll stamp you in for 30 days if you're from a visa exempt country.
An application is only needed if you're from a Visa on Arrival country where you apply for and pay for a visa for 15 days.
when was the last time ? I come every 2 weeks and they have been asking for at least 6 months. Previously they had the tm6 and never asked for boarding pass
you were lucky then because they have been asking at bkk and asked everyone in front of me for their boarding pass as well, pain in the ass as some have on their phone and have to hunt for it.
