Do I need to apply separately for my daughter as a dependent on the DTV visa, and what are the associated costs?

To apply as a dependent (me and my daughter), do we need 2 extra process or it is done all at once after the DTV holder is complete?

It’s also 400,00 dollars more? (For each person or both of us?)
