My husband and I are going to be applying for the TR Single visa and hoping to extend it the additional 30 days upon arrival. Can this be done in Phuket or does it have to be done in Bangkok?

i doubt you can immediately apply for the 30 days extension upon arrival..need to wait until you get closer to the end date.

But officially you can apply for your extension:

If you have Tourist visa: 15 days after you enter the Country;

If you have a visa exempt 30 day stamp: the same day you enter the Country.

Officially she can get an extension of a Tourist Visa 15 days upon arrival. But some Immigration could not respect this rule

You and your husband can apply a single entry Tourist Visa only in a Thai embassy or consulate outside Thailand, then you can get a 60 day stamp on arrival and you can get a 30 day extension at any local Immigration office paying a fee of 1900 baht.

A single entry tourist visa is VALID for 90 days from the date it's issued <- meaning you have that long to enter thailand and get stamped in for 60 days.

I'd just apply for it as soon as you can.

You can do this at any Immigration office. if you stay in Phuket you can do it there.
