TLDR : Answer Summary
For the 30-day visa extension at MTT, an appointment is generally recommended, but walk-ins are usually accepted as well. The experience may vary by location, and recent visitors report success without appointments.

Was just there today... Mostly walk and were there. Should be no problem without appointment

If you've entered thailand recently on either a 60 day tourist visa or a 45 day visa exempt entry and want to get a 30 day extension, they are asking you make an appointment but they will accept walk ups.
