TLDR : Answer Summary
For a 30-day visa extension in Thailand, you need to provide photocopies of the following: the main page of your passport (with your photo), your tourist visa page, the last admitted to stay stamp, and your TM6 card. Additionally, you'll need to fill out an application form and pay a fee of 1900 Thai Baht.
