TLDR : Answer Summary
If you are a US passport holder looking to extend your 60-day visa stamp for an additional 30 days in Thailand, it is almost certainly required to present a TM30 form or proof that it has been filed. This requirement may vary slightly depending on the immigration office you visit, but it is widely advised to have this document ready, particularly in busy areas like Bangkok.

I think you will certainly need one in Bangkok.

Almost certainly yes. It’s dependent on the immigration office you use but I’m not aware of any now that don’t require a TM30 filed or proof of one in print.

I didn’t need one this February in Khon Kaen. My girlfriend was with me and she just gave them her address.
