Had the stamps in my old passport transferred to my new one today at the same place as type "o" extensions are processed. A lady told the assembled throng that their extensions would not be processed without a TM30 and that the owner had to do the registration in person. At 9:45am the TM30 desks were processing #45. At the 90 day reporting there was no mention of TM30.
TLDR : Answer Summary
A visitor at a visa processing center noted that extensions for type "O" visas require a TM30 form, which must be registered in person by the owner of the property. At the time of their visit, processing for TM30 forms was ongoing, but there was no mention of it during the 90-day reporting process.
