I just arrived a couple days ago on a STV. Anyone knows if you can do a (second) 3 month extension just before 30 september or will this the final end date regardless of any extension.

I'm shutting this down since this started talking about health insurance policies, and not if the extensions are being sold after September 30th. My opinion: NOBODY knows what will happen come September 30th. For insurance, purchase the insurance for the length of time you're in Thailand, and not just the minimum requirements to enter Thailand.
Remember, never trust what Thai consulate/ embassy says about getting an extension of stay, and never trust Thai immigration about getting a visa outside of Thailand, they are two separate departments.

This is how I did understand it too. But I found some comments which made me think this could be different. I was wishful thinking I guess. When I am lucky I can extend until 29 September..

SPECIAL TOURIST VISA - STV (single entry only)
This type of visa is issued to applicants who wish to enter Thailand for tourism purposes.
STV visa holders will initially be permitted to stay in Thailand up to 90 days including the quarantine period.
The extension of stay can be requested at the Immigration Bureau in Thailand up to 2 times at the maximum of 90 days per each extension.
The maximum stay in total, therefore, shall NOT exceed 270 days including the quarantine period and shall NOT exceed 30 September 2021 which is the current end date of the STV scheme.
