TLDR : Answer Summary
The user currently has a 90-day tourist visa consisting of a 60-day visa and a 30-day extension. They are inquiring whether it is possible to obtain another 30-day extension in Koh Samui after having made prior extensions previously. Based on community insights, it is clarified that if the user has not yet extended their current tourist visa, they can visit the immigration office in Koh Samui and obtain a one-time 30-day extension.

sorry for the Dutch language. it is clear. She entered with Visa Exempt and Extended it, than left and came back with Tourist Visa and Extended this, left again and now here on Tourist Visa. She will visit Immigration for the Extension.
As I understand: she came in with a Visa Exempt stamp and did the Extension of Stay, left Thailand came back with Tourist Visa and did the Extension, "visited immigration twice." Left Thailand and came back with new tourist visa, so she can go to Immigration for an Extension.
You can only do the Extension once on every new entry or new Tourist Visa. You wrote that you're here with a tourist visa (60day + 30 days extension), the question is did you or did you not make already an extension on your last visa.
