I am currently a US passport holder in Thailand I arrived on a 90 day type O. Visa, I am wondering, can I extend that while I am in Thailand or do I have to leave and come back in to get 60 more days? It is single entry visa.

TLDR : Answer Summary
A US passport holder currently in Thailand on a 90-day Type O visa cannot extend this visa while remaining in Thailand. The options for extending the stay are limited, and the only available extension is a year-long extension of stay if married to a Thai national or having a Thai child. Alternatively, to obtain 60 additional days, the individual must leave Thailand and re-enter on a new 60-day visa exemption.

The only extension available is the one year extension of stay, unless you’re married to a Thai or have a Thai child and can apply for the 60 days extension visiting Thai family.

You’ll need to leave and re-enter on a new 60 days visa exemption if you don’t want to apply for an extension.
