Regarding a TM30. This is a notice sent from the property owner of the location you're staying at sent to the immigration office. The property owner, and not you, file this document.
For hotels, ask them for a print out of the TM30 they submitted online. It should be the middle picture of the attached file. If you're staying with family or friends, you can use any one of the three options.
AirBnBs GENERALLY do not submit a TM30, because technically they can not do short term rentals.
If a TM30 is not submitted, either:
1. YOU (the person doing the extension of stay request) will be fined. The fine for Bangkok immigration is 800THB, at most other immigration offices is 1,600THB, and a max of 2K THB.
2. The immigration office will refuse to process you request.
