I stay in Nonthaburi and used to go to Nonthaburi immigration office for extensions.

For the emergency extension using the embassy letter do I go back to Nonthaburi immigration or do I have to go to Muang Thong Thani (both of which are in Nonthaburi)?

you use the immigration office in your area which IS the original Nonthaburi office.. The Muang Thong Thani location is for Bangkokians
